From GuildWiki

The guild and Hero Battle ladders are the official rankings of guilds and players for GvG and Hero Battles, respectively. Guilds and players are ranked by their ratings which are modified by playing automatic rated matches and automated tournaments. To appear on the ladder, a guild or player must have played at least one match and have sufficient rating to place in the top 1000.

From the RuneScape Wiki, the wiki for all things RuneScape

Ladders are used to go up and down to the next or previous level of the RuneScape game. This is a very common piece of scenery found through out RuneScape. They come in many varieties; ghostly (such as the one at the entrance to Damis' Lair), broken, and regular (commonly found almost everywhere).

Trivia

  • There is a ladder that can be seen from the Void Knight's boat in Port Sarim. This ladder is located to the north east of the boat and can not be climbed and seems to lead into the ocean. The examine text for this specific ladder reads, "I'm not allowed to use this.". This ladder is an easter egg not commonly known.
  • There are many 'unusable' ladders throughout RuneScape, many of which can be found at ports and on the docks. Among these unusable ladders is the two ladders in the bank at Tutorial Island. When you try to climb up it, "Nothing interesting happens".
  • There was a glitch where after players had gone up a ladder, they would not be able to get down. They would also be able to roam anywhere.

From TibiaWiki

Ladders are non-moveable items that can be used to go up a floor or down a floor. To go up a floor, "use" the area at the base of the ladder (where the ladder meets the ground). To go down, simply walk on to ladder's hole. Most ladders look like this:

Though they are rare there are trap door ladders, that you may only go down (Like some holes in the maze of lost souls), but not up, even by using a rope. You can tell these ladders from regular ones, because the ladder on the trap door ladder looks "sawed off" and you see only a small (unusable) ladder segment.
