Heaven was located on the edge of Human space in the 2560s. It had a 23 hour day and was occupied primarily by a mix of Humans and Draconians. During this time it was used as a mutual burial site for both the Draconians and the Humans (following their conflict with the Daleks). In 2570 the Hoothi attacked Heaven, reanimating the bodies there. (NA: Love and War)
Later the Daleks used Heaven as a base of operations whilst posing as the leaders of the Fifth Axis, Bernice Summerfield returned to Heaven with Jason Kane to retrieve Bernice's father (Isaac Summerfield) from the Daleks' base there. (BFBS: Death and the Daleks)
Heaven is a paradise like location in many religions and the mythology of certain cultures, it is often a locale in the afterlife.
In Rajathan culture heaven is of equal status to the underworld and Rajathan monuments have ceremonial entrances into both. (TNG comic: "Captain's Pleasure")
The concepts of Purgatory (a soul between salvation and condemnation) and Perdition (hellbound, but a deed of sacrifice may grant entrance into Heaven) have also been addressed by many religions-including Earth's. Including the ideal of death not being the end of life; as it neither ends nor begins. Only the continuation of an afterlife, whether by re-incarnation, or more celestial in nature.

Heaven is a realm that exists only in the afterlife. It is where the souls of the just retire to when they pass away. Those who have lived a life of sin may find themselves wandering the limbo realm of Purgatory, or if they were truly evil enough, may even find themselves damned to Hell.

Heaven is the paradise available in the afterlife according to Christian theology. It is also the home of The Boss, Daniel, Glenn, and assorted angels in It's a Very Merry Muppet Christmas Movie. This version of Heaven consists largely of a shining white office space, full of cubicles and computers where angels do their work beneath a cloud ceiling. A door leads to the Boss' private sanctum: a pastoral valley full of flowers, green grass, a jurassic butterfly, and a couch strategically located to view the affairs of the world (or SpongeBob).
A more traditional Heaven is glimpsed in The StoryTeller episode "The Soldier and Death".
The Dinosaurs episode "The Last Temptation of Ethyl" features a version of Haven, referred to simply as "The Afterlife." Ethyl is transported there after a near death experience but is returned to Pangaea since it's not yet her time. After Ethyl starts "pushing the Afterlife," she returns a second time and Grandpa Louie warns her that she could go somewhere "not so nice," a personal hell consisting of dozens of Earl clones.
In an outtake from Letters to Santa, Kermit walks through his apartment door into a stage light and says, "Oh look, it's heaven!"

Heaven was a planet in another dimension of space and time that housed Yahweh and the Kingdom of Heaven, sometimes referred to as the Kingdom of God. When a good and righteous person died, their spirit was taken to Heaven where they lived in peace for all of eternity. If someone was not a good person in life, their spirits were taken to planet Hell where they would be condemned for seven years before being allowed into Heaven. In the case of exterme evils, condemnation to Hell would be eternal.
