From GuildWiki

An Afflicted Warrior, a typical Afflicted.

The Afflicted are cursed creatures that plague the land of Cantha. They look like a grotesque misshapen pile of human organs (and body parts of other creatures) shambling about. Some are more human-shaped than others. Most of them appear to have large, swollen tumors on their bodies.

In truth, the Afflicted are living creatures - animals, humans, plants - that have gotten too close to the malevolent spirit of Shiro Tagachi. The disease is not contagious, and only spreads through contact with Shiro (though in the Gyala Hatchery mission, the Luxons seem to think otherwise). After defeating their incoming hordes in the Eternal Grove mission, the Kurzicks and Luxons proceeded to burn the corpses of the fallen Afflicted.

The Afflicted retain the combat strengths of their former selves, but their powers are altered and augmented in terrible ways by the Affliction. Upon death they explode in a putrid manner causing high damage in an effect called Afflicted Soul Explosion.

The Afflicted can be found throughout Cantha, on Shing Jea Island, in the narrow alleys of Kaineng City and even making assaults in the Echovald Forest and the Jade Sea. In any area they are found, they will be hostile to all other creatures except Am Fah and Shiro'ken.

Notes

It appears that some afflicted are hostile to Am Fah while others are not.

Trivia

  • A few Afflicted bosses appear to be former students of Shing Jea Monastery. There are also a large number of Afflicted bosses with the same names as Shing Jea students, but different professions. See Category:Afflicted Bosses.
  • Unlike most bosses in Cantha, Afflicted bosses don't drop green items.
  • The Afflicted resemble the Flood in the Halo video game series.

From the RuneScape Wiki, the wiki for all things RuneScape

An afflicted woman's face.
The Afflicted are villagers of Mort'ton who were cursed by Lord Lowerniel Vergidiyad Drakan's evil gust of wind.

They speak nonsense to the player if talked to.

The Afflicted may be cured temporarily by either of the two serums, Serum 207 or Serum 208. After curing the afflicted, the cured villager will give the player rewards of various types, many related to making the cures. The Serum 208 yields better rewards from villagers.

These are also extremely useful for lower level players attempting barrows, who have ran out of food, as if you bring 3 bones to peaches tablets each trip, it can save lots of time, considering peaches heal 8hp, and bones are a 100% drop.
